In consideration of participating in cricket matches, training and associated events ("Activities") organized and/or supervised by the Five Star Cricket Club, I hereby acknowledge, agree, and confirm the following:

1 - There are inherent risks associated with the Activities that may result in personal injury (even of a serious nature) to participants. I/we fully accept and agree to bear these risks.

2 - I/we fully accept and agree to bear these risks. I understand that should personal injury occur, I will not be covered by the Five Star Cricket Club Insurance.

3 - I, the undersigned, declare that the above information is correct and that I have read and understand the conditions of registration to the Five Star Cricket Club and agree to follow them.
